Denture Care-Adhesives

Denture adhesives are only for loose fitting dentures.

There is a wealth of clinical evidence to support the fact that denture adhesives can enhance the performance of all dentures, even well fitting. Denture performance is enhanced in terms of retention and stability due to the increased hold an adhesive provides. Clinical evidence shows that with an adhesive, patients can bite harder without dislodgment and enjoy a wider range of foods.

Denture adhesives equal poor fitting dentures/admission of poor dentistry.

A denture however well made and well fitting can never replace the strength, mechanical action and strong biting surface of natural teeth. Therefore, the performance of a denture will never be equal to natural teeth, as patients only retain up to 1/3 of their original bite force. The retention of a denture is based on suction, which is dependent upon the shape of the mouth and the saliva produced. Everyone is different physically and all can benefit from the advantages of using an adhesive.

An adhesive will bring physical and psychological support to patients, and will enhance the performance of ill and well fitting dentures.

A denture adhesive is only for temporary/short-term use while new dentures are being constructed.

An adhesive can indeed be useful for temporary support for example when a patient is waiting for a new denture or a repair. However, adhesives can also offer patients confidence and physical advantages in long-term use.

Denture adhesives are only for patients with special needs.

Adhesives are a useful aid to denture retention and stability for all patients. They are particularly useful for patients with special needs e.g. poor neuromuscular control or dementia, but can offer advantages to all denture wearers. Clinical research has proven that use of a denture adhesive brings physical benefits, psychological benefits and support to patients even with well fitting dentures.

Denture adhesives are not necessary.

Up to 85% of patients believe their dentures will be as good as their natural teeth. Only up to 1/3 of natural biting force is retained with dentures and therefore dentures can never be as good as natural teeth. An adhesive is a useful aid to retention, stability and eating for all patients, even those with well fitting dentures. They also provide a protective layer cushioning the gums for extra comfort and help prevent rubbing. Adhesives also bring psychological support making patients feel confident and self-assured.

Denture adhesives are messy to use/my patients don't like them.

Today's adhesives are not what they used to be. They have been researched and developed, and the delivery mechanisms have been modernized to meet patient needs and requirements. Denture adhesives have evolved from the first generation of products using Karaya Gum and second generation CMC-Polyox formulations to today's Gantrez Salt products. The Gantrez Salt formulations have a firmer texture and stronger adhesive action, therefore they are less likely to ooze from under the denture plate. PoliGrip provides a range of product formats to provide customers a choice to suit all tastes and needs. Not all patients will like them, but it is important to give them an informed choice.

My patients don't like the taste.

Taste is a very subjective consideration. Often flavors such as mint can polarize patients' views. There are a range of products available, offering different strengths of flavor, such as Ultra PoliGrip Hold and Original, which have a mild mint flavour, and PoliGrip Free which does not contain artificial flavours.

I don't agree with recommending adhesives until a patient mentions them.

Tooth loss is a serious life event and patients need to be listened to and counseled. The recommendation to use an adhesive, up front at the outset of receiving a new denture not only provides the patient with added security but also confidence and peace of mind. If advised or recommended afterwards, once the patient has cause to question the fit of the denture this can been seen as an excuse for poor dentures. By discussing the use of an adhesive at the outset, the patient is more likely to feel positive about their dentures and confident that they have received the best treatment.

How can adhesives help prevent food particles from getting trapped under the plate?

Adhesives, when in contact with moisture (saliva) typically fill any space between the denture and denture bearing area. In so doing, the adhesive creates a seal, which physically blocks bits of food.

Adhesives are too expensive for my patients.

Adhesives are manufactured with the highest quality ingredients and to exact quality control specifications and Poligrip is a premium quality product. Poligrip costs approximately $5.00, which can last anywhere between 4 weeks and 3 months, depending on the frequency of usage. This cost is in line with many other health and beauty aid products and is cheaper than many prescriptive medicines. Given the profound impact and benefits an adhesive can offer, it is a relatively small price to pay. It is also important to note that GSK adhesives are competitively priced.

Denture adhesives interfere with the natural changes in the mouth/can impair the neuromuscular system, important for retention and stability of the denture.

All GSK adhesives are soluble. They are eventually lost from the denture and inevitably swallowed during normal wear along with food and drink. Research studies have concluded that there is no evidence to support the fact that the use of soluble adhesives affects natural changes in the mouth. Studies have also shown that there is an expected rate of change of bone support under a denture, with or without a denture adhesive and that the adhesive does not cause or affect this rate of change.

Studies have pointed out that the soft, soluble material of adhesives can not affect changes to the bone or bone resorption, due to the fact that the adhesive spreads over the denture as its material absorbs water, which provides an equal spread. This then provides a cushioning effect that acts as a protective layer and redistributes the stress of biting and chewing over the entire denture. This allows the load of the denture to be carried throughout the denture wearing area thus minimizing trauma and spots of localized rubbing or pressure. There is no clinical substantiation of soluble fixatives having a detrimental effect on bone structure or of irritation and studies have in fact concluded that adhesives can reduce irritation by re-distributing stress over the entire denture wearing area.
